SUMMARY DESCRIPTION OF WORK

The InSHAPE Coordinator is responsible for the supervision of InSHAPE Health Mentors, volunteer mentors, and interns. This position coordinates program administrative functions as needed. They also assist InSHAPE® members to develop and implement their personal wellness program. This position provides support, education, and motivation related to physical fitness, nutrition, and wellness. They provide individual and group activity instruction while working independently in community settings. This role is responsible for managing program schedules, planning and executing events, and maintaining smooth day-to-day operations. The coordinator builds and sustains strong relationships with internal departments and external community partners to enhance service integration and expand access to resources. The position requires strong leadership, organizational, and interpersonal skills, as well as a commitment to trauma-informed care and recovery principles.

Compensation: between $49,200 and $62,243 per year (salary is determined by total years of relevant experience)
Work Schedule: 40 hours per week

MINIMUM QUALIFICATIONS

  • Personal Trainer Certification
  • Current CPR and First Aid Certifications
  • Bachelors Degree in Health Science, Social Work, Rehabilitation, Psychology, Health Promotion or related field.
  • A commitment to decreased health disparity, and social stigma for individuals with mental illness.
  • One or more years in supervisory experience

Essential Duties and Responsibilities

  • Provides supervision of InSHAPE health mentors, volunteers, and interns as well as the Peer Center Nurse.
  • Tracks program data and reports to necessary agency requests.
  • Promotes and maintains community partnerships to increase awareness of mental and physical health impact in community outcomes.
  • Coordinate and manage all InSHAPE programming, including developing, scheduling, and implementing events, support groups, workshops.
  • Collaborate with agency leadership to evaluate program performance, identify operational improvements, and support program growth in alignment with agency strategic priorities
  • Manages a caseload of approximately 10 clients to establish and implement individual InSHAPE® plans.
  • Provides community-based support services to ensure implementation of client’s health goals
  • Works collaboratively with clients and program staff to design and support individual member plans.
  • Provides continuing reassessment of exercise, nutrition and healthcare goals.
  • Advise participants about important safety concerns and demonstrate exercises or maneuvers as needed that can be performed at the gym or in the community.
  • Encourages member participation and movement along the continuum of wellness related lifestyle changes.
  • Facilitates conversations with clients to build knowledge of the connection between physical health and mental health.
  • Administers fitness assessments as needed and records data.
  • Provides instruction and facilitation for group activities.
  • Works with clients to identify and establish wellness support system outside of InSHAPE® program.
  • Utilizes technology to provide client services when appropriate and completes required agency documentation within electronic health record.
  • Displays a courteous and caring attitude at all times to clientele, volunteers, and visitors of Heartland Family Service.
  • Cooperate and collaborate with program area staff, volunteers, and other Agency staff.
  • Abide by all specific program and Agency procedures, policies and requirements.
  • Develop personal and program related skills through participation in internal and external training opportunities.
  • Strive to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • Essential functions of this job are to be performed on company physical work site
  • Perform other program related duties as assigned.
